As a courtsey to our patients our office will coordinate between any medical and dental insurance, in-network or out-of-network, provide any documentation needed prior and post surgery, as well as submit and track all claims post surgery. 

On average it can take up to 90-120 days for full processing of both medical and dental insurances. We do require patient payment the day of the procedure and any charges from our office after insurance coordination are the patient's responsibility.

Your insurance coverage is a contract between you and your insurance company. It is not possible for us to provide services on the basis that your insurance will pay all charges because plans vary greatly. Often leaving a portion of payment to be responsible to the patient if not at times all costs.

For this reason, all insured patients will have a deposit requirement of 60% due on the day of surgery. You will be informed of this cost approximately one month prior to your surgery date. If you are unable to obtain the dollar amount needed by the time of the procedure we are able to do a pre-payment plan to assist in this need.

We will submit to in and out of network insurance carriers. If in the chance that there is an over payment a check will be issued to you from our office.

All consults, and any imaging, will be out of pocket, for those that are insured these will be submitted to insurance after the consultation is complete and future treatment plans are discussed.

Medicaid, Medicare with or without supplements, and self -pay patients will pay full amount at time of check in, however we will supply a CMS 1500 form prefilled out for you to mail to your insurance provider and obtain direct reimbursement after your treatment upon request.

We do ask to avoid any misunderstanding on available coverage or in-network vs out-of-network availability that the patient also call their insurance provider to discuss their coverage options prior to scheduling. If needed our office is happy to provide treatment plans to patients prior to their appointments for verification or a pre-authorization if requested.

For any payments that may be required after insurance or for those who may not have insurance coverage we do accepts personal checks, cash, all major credit cards, HSA/HRA plans, and Care Credit - to see what you qualify for through Care Credit you can apply at: (https://www.carecredit.com/) .

The cost of your treatment may vary, depending on your individual needs and treatment plan, for the most accurate personalized estimates imaging and consulation are needed.
